Tendon pain can become frustrating when symptoms repeatedly return with running, lifting, gripping, jumping, or other activities. Tendinopathies often develop gradually and may persist when the demands being placed on a tendon exceed its current ability to tolerate those loads.

At Juke Chiropractic & Sports Rehab, we take an active approach to treating tendon-related conditions. Rather than simply resting the painful area, we evaluate the involved tendon, surrounding strength and mobility, activity demands, and training history to determine what may be contributing to the problem.

What Is Tendinopathy?

Tendons are strong connective tissues that transfer force from muscles to bones. Running, jumping, lifting, gripping, throwing, and many other activities repeatedly load these tissues.

Tendinopathy describes persistent tendon pain and changes in tendon function that commonly develop following repetitive or excessive loading.

Symptoms may include:

  • Localized tendon pain or tenderness

  • Stiffness after periods of inactivity

  • Pain when beginning an activity

  • Symptoms that temporarily improve after warming up

  • Pain during or following exercise

  • Reduced strength

  • Difficulty tolerating repetitive activity

  • Symptoms that repeatedly return when training intensity increases

Tendinopathy is not simply an inflammatory condition, particularly when symptoms have been present for an extended period. This is one reason treatment often needs to involve progressive loading rather than relying entirely on rest or passive treatment.

Common Tendinopathies We Treat

Tennis Elbow

Tennis elbow, or lateral elbow tendinopathy, causes pain around the outside of the elbow.

Despite its name, you don't need to play tennis to develop it. Repetitive gripping, lifting, weight training, racquet sports, manual work, and computer-related activities can all place repeated stress on the involved tendons.

Symptoms commonly occur when gripping, carrying objects, lifting with the palm facing downward, or performing repetitive wrist movements.

Golfer's Elbow

Golfer's elbow affects the tendons around the inside of the elbow.

Symptoms may develop from golf, weight training, throwing, climbing, repetitive gripping, or occupational activities involving the wrist and forearm.

Treatment typically focuses on progressively rebuilding the strength and loading tolerance of the involved forearm muscles and tendons.

Achilles Tendinopathy

The Achilles tendon connects the calf muscles to the heel and experiences substantial force during walking, running, jumping, and athletic activity.

Achilles tendinopathy may cause pain, stiffness, or tenderness along the tendon or near its attachment to the heel.

Treatment commonly includes progressive calf strengthening and tendon loading while gradually restoring tolerance to running, jumping, and other activities.

Patellar Tendinopathy / Jumper's Knee

The patellar tendon connects the kneecap to the shin and plays an important role in jumping, running, squatting, and explosive movement.

Patellar tendinopathy is particularly common in athletes participating in volleyball, basketball, running, and other activities involving repetitive jumping or rapid force production.

Rehabilitation progressively strengthens the quadriceps and surrounding lower extremity while gradually increasing the tendon’s ability to tolerate higher loads.

Proximal Hamstring Tendinopathy

Proximal hamstring tendinopathy typically causes discomfort near the attachment of the hamstring around the lower buttock or sitting bone.

Symptoms may be aggravated by running, sprinting, hinging, prolonged sitting, or stretching the hamstring.

Rehabilitation often involves progressive hamstring and hip strengthening while carefully managing positions and activities that place greater compression or tension on the irritated tendon.

Rotator Cuff Tendinopathy

The rotator cuff consists of muscles and tendons that help stabilize and control the shoulder.

Rotator cuff tendinopathy can produce shoulder pain during reaching, lifting, pressing, throwing, or overhead activity.

Treatment may include shoulder mobility work, progressive rotator cuff strengthening, scapular strengthening, and gradual return to overhead or athletic movements.

Gluteal Tendinopathy

The gluteal tendons attach muscles around the hip to the upper portion of the femur.

Gluteal tendinopathy commonly produces pain along the outside of the hip and may be aggravated by walking, running, stairs, prolonged standing, or lying on the affected side.

Treatment typically focuses on progressive hip strengthening while modifying activities or positions that repeatedly aggravate the tendon.

Plantar Fascia and Other Chronic Soft Tissue Conditions

Although the plantar fascia is not technically a tendon, persistent plantar fascia pain shares several characteristics with other chronic overuse conditions.

Shockwave therapy and progressive loading strategies may also be incorporated into treatment for persistent plantar fasciitis.

Other tendon-related conditions can involve the biceps, quadriceps, adductors, wrist, ankle, and other areas depending on activity and injury history.

Why Tendon Pain Can Become Chronic

One of the challenges with tendinopathy is that symptoms often improve temporarily with rest but return when normal activity resumes.

Rest may reduce the immediate demand on the tendon, but prolonged avoidance of loading can also reduce the tendon and surrounding muscles' ability to tolerate activity.

This can create a cycle:

Activity increases → tendon becomes painful → activity stops → symptoms improve → activity resumes → pain returns.

A successful rehabilitation plan attempts to gradually break this cycle by exposing the tendon to an appropriate amount of progressive loading.

When appropriate, Active Release Technique (ART) and other soft tissue treatments may be incorporated to address areas of tenderness, muscle tension, or restricted movement around the affected region.

These treatments are typically combined with exercise rather than being relied upon as the sole treatment for persistent tendon pain.

Progressive Tendon Rehabilitation

Progressive loading is one of the most important components of tendinopathy rehabilitation.

Early exercises may begin with relatively simple or controlled resistance. As the tendon becomes stronger and symptoms become more manageable, exercises can progressively increase in resistance, speed, and complexity.

Depending on the tendon and activity, rehabilitation may include:

  • Isometric strengthening

  • Slow resistance exercises

  • Eccentric and concentric strengthening

  • Progressive heavy resistance

  • Single-leg or single-arm exercises

  • Plyometrics

  • Running and jumping progressions

  • Gripping or throwing progressions

  • Sport-specific exercises

The exact progression depends on the tendon involved and what you ultimately need that tendon to tolerate.

Can I Continue Exercising With Tendinopathy?

Complete rest is not always necessary for tendon pain.

In many cases, activity can be modified while rehabilitation progressively increases the tendon’s capacity.

We consider how symptoms behave during activity, immediately afterward, and over the following day when determining whether training volume needs to be adjusted.

The goal is to find an appropriate amount of loading rather than simply choosing between "exercise through it" and "stop everything."

Returning to Sports and Activity

A tendon that feels comfortable during everyday activities may not yet be prepared for sprinting, jumping, heavy lifting, throwing, or other demanding movements.

For athletes and active adults, rehabilitation should eventually reproduce the types of forces the tendon will experience during your activity.

As strength improves, treatment can progress toward faster movements, heavier resistance, plyometrics, and sport-specific demands.

Our goal is not simply to make the tendon less painful. We want to rebuild its ability to handle the activities that matter to you.

When Should Persistent Tendon Pain Be Evaluated?

Tendon pain that has persisted for several weeks, repeatedly returns when activity increases, or prevents you from participating in normal exercise or sports is worth evaluating.

Sudden injuries involving a pop, significant bruising or swelling, immediate loss of strength, or inability to use the involved muscle normally may indicate a tendon tear or rupture and should be evaluated promptly.

For gradual or persistent tendon pain, an examination can help determine which tissue appears involved and develop an appropriate treatment and rehabilitation plan.
